Fill out the form below to pledge your support by committing to raise $1,000, or more, through your network of friends and family, in support of Trillium Health Partners (THP).
Secure Your Spot:

Skydivers will be scheduled on a first-come, first-served basis.
Skydiving Costs:
The total cost for the skydive is $350, payable on the day of your jump directly at Skydive SWOOP by credit card.

Fundraise for the Cause:
You can pledge a period of your convenience to meet or exceed your fundraising goal. Every dollar you raise will go directly toward supporting THP and helping the Muslim community contribute to the $5 million fundraising goal.

By participating in the skydiving event above, you acknowledge and agree to the following terms:
- Assumption of Risk: Skydiving is an inherently risky activity, and participation involves the risk of serious injury, including permanent disability or death. Participants voluntarily assume all risks associated with skydiving.
- Health and Fitness: Participants affirm that they are 18 years of age or older and in good health and physical condition to engage in skydiving activities. It is the responsibility of each participant to ensure they are medically cleared to participate in such activities.
- Release of Liability: Participants hereby release, waive, discharge, and covenant not to sue Organizer/s, its officers, directors, employees, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by participants, whether caused by the negligence of the organizers or otherwise.
- Indemnification: Participants confirm that they agree to indemnify and hold harmless Organizer/s from and against any and all claims, demands, actions, liabilities, damages, expenses, or losses, including attorney fees, arising out of or related to the participant’s participation in the skydiving event.
- Compliance with Instructions: Participants agree to comply with all instructions provided by the organizers, including safety briefings, equipment usage guidelines, and any other directives intended to ensure the safety of participants.
- Insurance: Participants acknowledge that they are responsible for obtaining their own insurance coverage, including health and accident insurance, to cover any potential injuries or damages arising from participation in the skydiving event.
- Governing Law: This disclaimer shall be governed by and construed in accordance with the laws of [Jurisdiction], and any disputes arising under this disclaimer shall be subject to the exclusive jurisdiction of the courts of Ontario.
